Google Says Pixel Watch Will Track Insulin Resistance Trends

Google said on August 12 that qualified Pixel Watch and Fitbit Air devices will add an Insulin Resistance Trends feature in September 2026. It will sit inside Health Guardian, a group of monthly summaries that flag changes in health and wellness signals.

The feature does not use a blood sample. Google says it analyzes several weeks of physiological data and is built on a sensor model trained with more than one trillion minutes of data from five million people who opted in. The company's footnotes set a firm boundary: this is a general-wellness feature, not a diagnostic tool or diabetes prescreener, and it should not be used to change medication or insulin doses.

The 0.80 result is not a watch-only result

The research behind Google's announcement is the WEAR-ME study, published in Nature. It included 1,165 U.S. adults with a median age of 45, median BMI of 28, and median A1c of 5.4%.

The study used HOMA-IR as its reference measure of insulin resistance. HOMA-IR is an index calculated from fasting glucose and fasting insulin. Against a HOMA-IR cutoff of 2.9, a model combining wearable data, demographics, and routine blood biomarkers reached an AUROC of 0.80, with 76% sensitivity and 84% specificity.

AUROC measures how well a model separates two groups across many possible cutoffs. A score of 1.0 is perfect separation and 0.5 is no better than chance. It does not tell us that a consumer watch alert is 80% accurate.

That distinction matters because the 0.80 figure includes blood biomarkers. In a separate validation group of 72 people, a model using wearable-derived features and demographics reached an AUROC of 0.75, compared with 0.66 for demographics alone. Adding wearable features to a model that already included demographics, fasting glucose, and a lipid panel raised AUROC from 0.76 to 0.88.

The study supports the idea that wearable data added useful signal. It does not support applying the blood-inclusive 0.80 result to the wearable-only consumer feature.

The validation sample was small and Google funded the study

The wearable-only comparison came from 72 people, much smaller than the 1,165-person main cohort. That makes it useful validation evidence, but not a final answer about how the released feature will perform across different ages, body sizes, health conditions, devices, and regions.

The financial relationship is also unusually direct. PubMed's conflict statement says Google funded the study. It lists 12 authors as Alphabet employees and two more as Google interns when the work was performed. The remaining authors declared no competing interests.

Company funding does not invalidate a peer-reviewed paper. It does make independent replication and post-release performance data especially important.

Where blood testing fits

Google says the feature does not provide real-time clinical readings and is not a prescreener for diabetes. The Nature study also did not train against a watch-based diagnosis. Its reference was HOMA-IR, which depends on measured fasting glucose and insulin.

A fasting insulin test provides a measured insulin value after fasting. A diabetes risk panel combines fasting glucose, A1c, and fasting insulin. Those results answer different questions from a wearable trend flag. Neither a watch alert nor one laboratory value should be treated as a stand-alone diagnosis of insulin resistance or diabetes.

What not to overread

The consumer feature had been announced, not released, when Google published its post. The paper validates the underlying modeling approach, but it is not an independent, real-world evaluation of the finished Health Guardian feature.

The restrained conclusion is that multi-week wearable data may add information about metabolic patterns. The evidence does not show that a watch can replace laboratory testing, establish a diagnosis, or direct treatment.
