1. Nature of Our Services

LevelPanel is an informational and ordering platform that connects consumers with CLIA-certified laboratory testing services provided by Quest Diagnostics and LabCorp. LevelPanel does not provide medical advice, diagnosis, or treatment.

Test results provided through our platform are for informational purposes and do not constitute a medical diagnosis. You should always consult with a qualified healthcare provider for interpretation of results and medical decisions.

2. Physician Authorization

All laboratory test orders are reviewed and authorized by a licensed physician in our independent physician network. This physician authorization satisfies the legal requirement for a physician order but does not establish a traditional doctor-patient relationship.

3. Eligibility

• You must be at least 18 years of age to use our services unless otherwise specified for a particular test.
• Our services are not available to residents of New York, New Jersey, or Rhode Island due to state regulations regarding direct-to-consumer testing.
• You must provide accurate and truthful information when placing an order.

4. Payment and Pricing

All prices are displayed in US dollars and are due at the time of ordering. We accept major credit cards, debit cards, HSA (Health Savings Account), and FSA (Flexible Spending Account) cards.

We do not accept insurance, Medicare, or Medicaid. Prices are subject to change without notice, but any price changes will not affect orders that have already been placed and paid for.

5. Refund and Cancellation Policy

• Orders may be canceled for a full refund at any time before a lab specimen has been collected.
• To request a cancellation, email [email protected] or call (480) 387-9420.
• Once a specimen has been submitted to the laboratory for processing, the order cannot be canceled and no refund will be issued.
• If a lab visit cannot be completed due to circumstances beyond your control (lab closure, specimen collection error by lab staff), contact us and we will work with you to resolve the issue, which may include reprocessing the order or issuing a refund.
• Refunds are processed to the original payment method within 7-10 business days.
• LevelPanel is not responsible for fees charged by the laboratory that are outside of the test purchase price.

6. Limitation of Liability

LevelPanel is not a laboratory and does not perform the actual testing. We are not responsible for errors in laboratory processing, though we will work with our laboratory partners to resolve any issues.

To the fullest extent permitted by law, LevelPanel shall not be liable for any indirect, incidental, special, or consequential damages arising from your use of our services.

7. Privacy

Your use of our services is also governed by our Privacy Policy, which describes how we collect, use, and protect your personal and health information.

8. Intellectual Property

All content on this website, including text, graphics, logos, and software, is the property of LevelPanel, LLC and is protected by applicable intellectual property laws. You may not reproduce, distribute, or create derivative works from our content without written permission.

9. Changes to These Terms

We reserve the right to modify these Terms of Service at any time. Changes will be effective immediately upon posting to this page. Your continued use of our services after changes are posted constitutes acceptance of the modified terms.
