What this test measures

IGF-1 (insulin-like growth factor 1) is a hormone that mediates most of the growth-promoting effects of growth hormone (GH) throughout the body. When the pituitary gland releases GH, the liver responds by producing IGF-1, which then acts on bones, muscles, and other tissues to promote growth and repair. Together, GH and IGF-1 are the primary drivers of normal skeletal and tissue development.

The test measures IGF-1 rather than GH directly because GH is released in short bursts and fluctuates dramatically based on sleep, exercise, meals, and time of day. IGF-1, by contrast, circulates at relatively stable levels that reflect average GH activity over time, making it a far more practical and reliable marker. Healthcare providers use the IGF-1 test to evaluate growth disorders in children, screen for GH deficiency or excess in adults, and monitor treatment in patients receiving growth hormone therapy.

Understanding your results

IGF-1 reference values are heavily age-dependent: levels are highest during puberty and adolescence and decline steadily through adulthood. For this reason, results must always be compared to age- and sex-matched reference ranges. Many laboratories report a Z-score alongside the raw value, indicating how far the result falls above or below the population mean for the relevant age and sex group. Healthcare providers interpret IGF-1 results in combination with clinical symptoms, growth data (in children), and sometimes additional stimulation tests to arrive at a complete assessment of growth hormone function.

What does a “Low” result mean?

Lower than normal levels of IGF-1 probably indicate growth hormone deficiency or insensitivity to GH. In children, this may be caused by a genetic disorder or brain disease. Low IGF-1 levels: especially when below the age-specific reference range: can point to growth hormone deficiency, GH resistance, malnutrition, or chronic illness. Persons with anorexia or malnutrition have low values of IGF-1, and malnutrition results in low serum IGF-1 concentrations, which recover with restoration of adequate nutrition.

What does a “High” result mean?

Higher than normal levels of IGF-1 may indicate gigantism in children or acromegaly in adults. These conditions are most often caused by a tumor in the pituitary gland. Elevated IGF-1 is often associated with conditions like acromegaly, usually due to excess GH secretion, often caused by a pituitary tumor. High levels may also be associated with other conditions affecting growth hormone production or certain medications that influence IGF-1 levels.
