What this test measures
Lipoprotein-associated phospholipase A2 (Lp-PLA2) is an enzyme produced by inflammatory cells within arterial plaques. Unlike hs-CRP, which reflects general inflammation from any source (infection, autoimmune disease, injury), Lp-PLA2 is specific to vascular inflammation, making it the most targeted blood marker currently available for detecting active inflammatory processes inside artery walls.
The enzyme's role is paradoxical. It is produced as part of the body's attempt to clear oxidized LDL cholesterol from vessel walls, but the byproducts of its activity, lysophosphatidylcholine and oxidized free fatty acids, are themselves potent inflammatory triggers that accelerate plaque growth rather than resolving it. This creates a destructive feedback loop within plaques. Lp-PLA2 is particularly valuable for identifying vulnerable plaques, the thin-capped, inflammation-rich deposits most likely to rupture and cause an acute heart attack or stroke. Elevated levels signal that this active, dangerous process is underway, even if standard cholesterol numbers appear normal.

Results are reported in nanograms per milliliter (ng/mL). What does a “Low” result mean?
